If not, contact your plans customer service representative and request a copy.\u00a0 This notice outlines changes in the plan\u2019s benefits, coverage, formulary, premium and\/or costs that will take effect January 1, 2021.\u00a0 This is your chance to determine whether your plan will meet your needs and be cost-effective in 2021 or check out other plans and make the switch by December 7th.
