Entities interested in becoming a PQRS qualified registry or QCDR for 2016 must submit a self-nomination to CMS. The self-nomination must contain the following: \u00a0a declaration of the entity\u2019s intent to participate in the program, the PQRS measures and non-PQRS measures (including their specifications, if the entity is intending to participate as a QCDR), and data validation plan as required by CMS.\u00a0 Acceptable versions of these deliverables must be received by the deadline to be considered for the qualification process.
