The individual can enroll in a Special Needs Plan (SNP) where a Medicare Advantage (MA) plan coordinates the care plan designed to provide targeted care and services to individuals with unique needs. There are three different types of \u201cSpecial Needs\u201d plans: Dual Eligible SNP (D-SNP), Chronic Condition SNP (C-SNP), or Institutional SNP (I-SNP).
