Medicare Advantage SNP for ESRD patients

In a recent study published by Health Affairs, it found Medicare Advantage special needs plans or SNPs, may help control healthcare spending for patients with end-stage renal disease (ERSD). In order to help coordinate care and cover their needs through alternative payment models, the Center for Medicare and Medicaid Services established a Medicare Advantage SNP for ESRD patients.
