Self-Nomination for 2016 Qualified Registries and Qualified Clinical Data Registries (QCDRs) Open Through January 31, 2016

Entities interested in becoming a PQRS qualified registry or QCDR for 2016 must submit a self-nomination to CMS. The self-nomination must contain the following: a declaration of the entity’s intent to participate in the program, the PQRS measures and non-PQRS measures (including their specifications, if the entity is intending to participate as a QCDR), and data validation plan as required by CMS. Acceptable versions of these deliverables must be received by the deadline to be considered for the qualification process. Self-nominations must be submitted electronically using a self-nomination form on JIRA prior to the 5:00 p.m. Eastern Time (ET) on January 31, 2016.
The 2016 Self Nomination User Guide is now available in the Criteria Toolkit. This User Guide provides basic step-by-step instructions on how to access the JIRA System, create and populate a JIRA Self-Nomination Form, submit self-nomination for 2016 PQRS, and manage and update information.
