Humana takes the next step with a new value-based care model

The insurance giant Humana announced it is launching a new value-based care model to support providers in identifying and addressing food insecurity, housing, social isolation and loneliness.  This “next step” for Humana will broaden the scope of patient care to more holistically improve health outcomes since social determinants of health are linked to an increased risk of chronic medical conditions.
