What is the appeal process for original Medicare?

If Medicare denies coverage for any service, you may appeal their decision. The appeals process involves five levels so if you do not win on the first level, you may move on to the second.  So what are the reasons for filing an appeal?  You may file an appeal for several reasons, including:  When Medicare, a health plan, or drug plan makes a decision that results in denial to cover or pay for a piece of durable medical equipment (DME), healthcare service or mediation, to change the fee that an individual must pay for DME, healthcare service or medication.
